Mousa Dembélé faces lengthy ban after appearing to gouge the eye of Diego Costa during Monday night's 2-2 draw at Stamford Bridge.
Both Chelsea and Tottenham have also been hit with FA charges for failing to control their players during an ill-tempered Premier League decider.
"It follows incidents which occurred in the 45th and 87th minutes and also at the end of the game at Stamford Bridge on Monday night [2 May 2016]" the FA said in a statement today.
"Both clubs have until 6pm on Monday 9 May 2016 to respond to their charge."
Players were involved in a number of incidents during the game as tempers flare and was most clearly evident in Dembélé's eye gouge.
He may be suspended up for five to six games, while the clubs will be slapped with six figure fines.
"It is alleged the player’s behaviour constituted violent conduct in circumstances where the standard punishment of three matches that would otherwise apply is clearly insufficient" the FA said of the gouge.
"The player has until 6pm on Thursday 5 May 2016 to respond to the charge."
A record nine Tottenham players were cautioned during Monday's game, the most of any of the same team in a Premier League match.
