Referee Robert Madley awarded a penalty which Milner converted.
Sadio Mane thought he had made it 2-0 with a side-footed volley but Adam Lallana was flagged for offside in the build-up.
Tottenham, who grew into the game, then found an equaliser with 19 minutes to go as Danny Rose controlled Eric Dier's cross to the far post and showed composure to slot past Simon Mignolet.
