John Terry says goodbye to Chelsea this afternoon as he plays his final game in front of a home crowd at Stamford Bridge.
The former England defender may also feature in the FA Cup final for Chelsea when they take on London rivals Arsenal, but took his chance to say goodbye to fans today in his programme notes.
"These are my last captain's notes at Chelsea," he begins. "Even writing that is difficult for me but I can't tell you how happy I am to be going out at the Bridge with a title celebration. I could not have asked for more after 22 incredible years at this great club."
Antonio Conte's side will be crowned Premier League champions this afternoon after securing the title away to West Brom earlier this month.
"I owe so much to so many people who have helped me throughout my career - my family, close friends and everyone who nutured my talent and made sure I had the best possible conditions to keep kicking on and improving."
He added: "I am and will always be extremely grateful for your backing from when I first stepped onto the pitch at Stamford Bridge for my debut in October 1998 right through to today. From the bottom of my heart thank you."
Chelsea can become the first Premier League side to win 30 games in a season with victory of relegated Sunderland.
